Hi Steve,

I can't find the LM354 in any of my data, sorry.
(Can find the LF354 and LM384 but I'm sure you know that!)

I do have pinouts and minor details on the TAA621A11 which
I found details of here on this RGP thread...
(may be compatible with LM354)
http://tinyurl.com/ysnme

TAA621A11
"Single Channel Audio Amplifier"
DIP14

1 Output 1 14 Supply Voltage
2 NC 13 NC
3 Ground 12 Compensation
4 NC 11 NC
5 Ground 10 Feedback
6 NC 9 NC
7 Null 8 Ripple Bypass

I searched through all my store of linear IC's and I don't
have one either.
www.usbid.com lists the TAA621A11 available from a few places,
but they may be pricey or high minimum order?
www.nteinc.com shows the TAA621A11 NLA, but TAA621 crosses to an
NTE1112, not sure if that is also compatible...

What board is it out of and what do you need to do?
It may not be too hard to drop another similar amp IC in its place...

Best regards,

Marc
