Usually mine will start in Star Trek, but it does this when something is
wrong. Actually, just about anything wrong anywhere has caused this, so I
would guess it could be triggered by transients or other power problems.

I would do a general check that all connections are tight, all the soldered
wires to the card cage fingerboard connector are good, etc.
