Frank,

Check for a waveform at the BU406d. If one is mssing, this can cause a
problem.

Also put those resistors on the board..

Check the 1n754a zener in the overvoltage circuit.

You can't install MC1 backwards ;) Maybe upside down, but it'd be tough
to solder to the board then.
