I think that most have at least heard about silpad, and many have seen them.

I think that the main reason that they aren't used as much is price and
availability -- after all, you can buy mica insulators just about anywhere
for pennies, where silpad is like a buck or so.

Further, from my experience it isn't the heat that kills the transistors,
but other components that blow and take out the trannies in the process, so
the increased thermal transfer doesn't help much.

I guess if you can measure a temperature difference between the transistor
body and the chassis, then it might help. I've never done this myself.
